Patient Education Order Form
The Urology Care Foundation provides free brochures and other patient education materials on urologic health to patients, caregivers, community organizations, healthcare providers, students and the general public, pending availability. Please complete the order form below, and the Foundation will ship our available materials to you, free of charge. Are you requesting materials to distribute at a community public health awareness event? Please be sure to promote your event, also free of charge, on our UrologyHealth.org Calendar of Events!
Are you looking for a brochure on a particular condition and do not see it listed below? Please email your thoughts and suggestions to info@urologycarefoundation.org. The Urology Care Foundation’s ability to produce patient education materials is dependent on support from our generous donors. Please note, due to an overwhelming demand for our free patient education materials, our order form will only accept one request for up to 50 copies of each item from each user one time per month. If you are hosting a large event or would like to order more than 50 free copies of each brochure per month to distribute to a higher volume of patients, please contact us at info@urologycarefoundation.org, and we will do our best to accommodate your request.
