Preparation of the Application Materials
- Complete the online sections of the application.
- Download the Application Agreement Form.
- Prepare the application package, which includes the following documents:
- Application Agreement Form with all necessary signatures.
- Research Project Abstract (no longer than one page)
- Applicant’s curriculum vitae or biosketch and a statement of future career plans. The Applicant may also submit an NIH-style biosketch and attach an additional page for career plans. (Total four pages maximum).
- Letter of support for Applicant from the Residency Program Chair
- Letter of support for Applicant from the Urology Department Head
- Letter of support from a previous mentor or medical school chair
- Description of the research project(s) that Applicant will be working on (two pages maximum). The description should clearly state how the Applicant would allocate the research/clinical time for the duration of the award period.
- Description of research facilities (one page maximum)
- A detailed budget detailing the projected expenditures during the award period
- A current NIH-style biosketch of the Residency Program Chair (four pages maximum)
- A current NIH-style biosketch of the Urology Department Head (four pages maximum)
- Assemble all documents in the above order and scan them into a single .pdf file.
- Submit the application package electronically.
The application package must be SUBMITTED no later than Friday, February 17, 2012 at 5:00 pm LOCAL TIME. Applicants should allow ample time for compiling and submitting materials, including the electronic upload process. Late submissions will not be considered.
Lay Language Statement and Photo for UrologyHealth.org
All Applicants are required to submit a short paragraph (5 – 6 sentences) lay language statement about their research project and headshot. The applicant thus gives permission for the information and his/her photo to be posted under the current AUA Foundation-supported research at www.UrologyHealth.org and in press releases and other promotional materials for the program. When taking the photo, please use the highest quality setting on the camera. Depending on the model of the camera, this should be at least 960 x 1280 pixels or 3 mega pixels or 300 dpi at 4" x 5". NOTE: Please do not use camera phones.
Statements and photos will be due no later than February 17, 2012. The statement and photo should be submitted via e-mail to grants@auafoundation.org. This information will only be used for promotion purposes described above. These will not be shared with reviewers and will not impact the review process or decisions of the reviewers.
