Reporting Requirements
Final Report and Personal Statement
Within 30 days of the end of the research training period, or by August 1, 2013, whichever comes first, the awardee shall submit a report electronically to the AUA Foundation Office of Research. The report should detail:
- Summarize the main focus of the research and clearly state the findings. Specify how the research results met the objectives established in the application.
- A personal statement from the awardee on their experience during the award period and how has the funding provided by AUA Foundation helped the awardee's overall professional growth (i.e. career opportunities, potential research focus). Does the Awardee plan to continue within the field of urologic research?
- A final financial statement detailing expenditures during the award period
Residency Program Chair Report (One page)
Under separate cover, the Residency Program Chair will submit a confidential statement evaluating the resident’s performance and detailing the future career plan for the scholar.
Other Requirements
- Notify the AUA Foundation of any personnel, location, or project status changes that occur after the award announcement or during the award period.
- Provide a copy of the current IRB approval letter if activities involving human subjects are planned during the proposed research.
- Provide a copy of the current IACUC approval letter if activities involving vertebrate animals are planned during the proposed research.
Failure to adhere to these requirements and/or provide appropriate documentation may result in a delay in payments, repayment of any or all AUA Foundation funds previously paid to the institution, and/or termination of the award. Unless approved by the Institution and the AUA Foundation, failure to complete the duration of the research period may result in repayment of funds to the AUA Foundation.
