When a patient moves from one medical team to another, it's called "Transitional Care". The goal is for a patient to have a smooth change, with full access to care at all times. With pediatric urology, children will one day grow too old for their pediatric urologist. They will benefit from the care of an adult urologist. This fact sheet describes that transition.
